RESUMO

In 1999-2000, puncture cytological diagnosis was made in 134 patients. Enlarged lymph nodes (LN) were detected in 114 patients. The enlargement occurred due to nonspecific inflammation (38.6%), common hyperplasia (26.3%), tuberculosis (18.5%), metastases of cancer (14.9%), sarcoidosis (1.7%). LN tuberculosis was a frequent finding in adult women--in 2/3 of all tuberculosis cases. In children it was rare (3.7%). LN enlargement was caused by cancer in divided by of the cases. LN tuberculosis was detected in adults 5 times more frequently than in children. M. tuberculosis were detected bacterioscopically in tuberculous lymph nodes puncture biopsies only in 6 examinees (28.6%). The Mantoux test appeared unreliable in diagnosis of peripheral LN tuberculosis. Tuberculous LN located under the mandible or on the neck. Specific non-tuberculous processes, primarily malignant tumors, were revealed in enlarged parotid and supraclavicular LN in half of the cases.

RESUMO

Screening for chronic hyperplastic laryngitis (CHL) has been performed among 70,000 workers and peasants of the Moscow Region. CHL was detected in 355 examinees (0.47%). The data were analysed for 166 CHL patients (age 31-50, 87 males, 29 females). 8 patients had laryngostenosis but tracheostoma was indicated only one of them. Malignant transformation occurred in 3 patients. Laryngeal lesions were symmetric in 36 patients, unilateral with separate inflammatory infiltrates clinically similar to tumors were seen in 45 patients. Local forms of CHL may look as separate small nodules, contact lesions in the region of vocal processes of the arytenoid cartilages, and diffuse inflammatory edema of the vocal cords. In rare cases inflammatory tumor-like laryngeal lesions in CHL depend on local flock of lymphoid elements--lymphoid hyperplasias. A cytological study in CHL is effective in identification of the signs of dystrophy, keratinization and epithelial cell atypia as well as the presence of microflora.

RESUMO

Data are available on 14 patients with sarcoidosis of the face and upper respiratory tracts (11 females and 4 males, age from 29 to 71 years). 4 patients had isolated ENT lesion. In 3 cases, sarcoidosis was found at histological examination of the material removed in the course of surgery for chronic tonsillitis, recurrent purulent cyst, pyopolypous maxilloethmoiditis. Pain at the site of skin and mucosa affection was minor, ulceration was absent. Standard treatment was effective. 12 patients were followed up for 3 to 15 years. Good results were achieved in 2 patients, the rest were cured with a picture of mucosal atrophy in the lesion zone.

RESUMO

In 1989-1998 the authors treated 50 patients (45 males and 5 females aged 21-68 years, mean age 46.2 years) with tuberculosis of the lungs and larynx. Pulmonary lesions were for the most part disseminated (58%) or infiltrative (24%). One patient had laryngeal tuberculosis combination with cancer of the laryngeal pharynx. Progression of the disease occurs frequently because of otorhinolaryngologists who make diagnostic errors when consulting patients who seek primarily their advice (60% of those with laryngeal tuberculosis) complaining about throat pain and hoarseness. In obscure cases, an attempt of antituberculosis treatment should be made.

RESUMO

Two cases of rhinophyma are described. Clinical and cytological features are specified. Rhinophyma can be diagnosed cytologically by hyperplastic cells of sebaceous glands. However, cytological diagnosis should also take into consideration clinical picture of the disease as the above cells can be encounted also in hyperplasia and/or adenoma of the sebaceous gland as well as basalioma with sebaceous differentiation.
